Dear team, I had an egg retrieval today and 13 eggs were retrieved. We don’t yet know how many were mature or how many could be fertilized, but we’re hoping for a day-5 transfer on Saturday. So far, I’ve only had frozen embryo transfers, and my body has a hard time absorbing progesterone into the bloodstream, so I’ve always had to take 400 mg vaginally three times a day and also inject Prolutex once a day starting one day before the transfer. How does this work for a fresh embryo transfer? My doctor was a bit unsure and said that, just to be safe, 400 mg vaginally twice a day should be enough? How would you handle this?

With 3 doses of 400 mg and Prolutex, I’ve only ever managed to get just under 10–11 μg of progesterone in my blood; with vaginal suppositories without Prolutex, unfortunately, only 5–6. Now I’m worried that we’re dosing too low again. But unfortunately, I don’t know how differently progesterone behaves in a fresh vs. a frozen embryo transfer...

    Dear Leonard,

Vaginal progesterone can usually be combined well with Prolutex, but vaginal absorption has its limits. A dosage of more than 400 mg three times daily is generally the upper limit when administered in combination with Prolutex and oral progesterone.

It should be determined 1–2 days before the transfer whether a fresh transfer is advisable or whether a cryotransfer should be performed and the embryos frozen.

    Sincerely,

    Dr. Miranda Sanz.

I live very far from the clinic, which is why they don't recommend that I have my blood drawn.

That’s why my doctor said I should just take Cyclogest vaginally twice a day starting today as a precaution and start taking Prolutex on Friday, the day before the transfer.

I’m worried that our dosage is too low again. I’ve already “lost” four embryos because my levels were around 5 ng on the day of transfer every time...

I was hoping that the fresh embryo transfer might go better because of my body’s own progesterone.

    That is indeed possible.

But without a blood test, you can’t know for sure.

It’s strange that you can’t check this, even if you don’t go to the clinic.

Most of our patients come from abroad and have the test done at a local lab near their home. Can’t they prescribe the blood test so you can have it done anywhere?

Unfortunately, I can’t help you further, but I wish you all the best for this transfer and hope that your progesterone level is sufficient.

    Best regards,

    Dr. Miranda Sanz

Leonard You could simply have your progesterone level tested at your gynecologist's office or at a direct-to-consumer lab (e.g., Bioscientia or similar) near you—at your own expense, of course. Or your primary care physician could draw blood for you and should be able to order the test.
