    Hello

Is immunological compatibility always taken into account when donating eggs in Spain? Are the KIR and HLA of the woman and the donor always looked at? What about the man?

In the case of increased killer cells in the recipient, how is Intralipid administered? In Germany, intralipid is always given a few weeks before transfer, then on the day of transfer and then again a few weeks afterwards.
This seems to be different in Spain.
I have also read here that Intralipid is not well tolerated if you are taking Omega 3. Is that true?

    Best regards

    Dear SoZi

At our clinic, we test the HLA-C value of all egg donors. This can be taken into account during the selection process, in which case the values of the patient and her partner are also determined (patient: KIR, HLA-C; partner or sperm donor: HLA-C; egg donor: HLA-C). This is particularly recommended for patients with implantation disorders or multiple miscarriages, but can be performed on all patients.

With regard to immunological treatment, each patient is specifically examined to determine which treatment - in consultation with an immunologist - is recommended. Intralipid, among other things, can be administered.

    Yours sincerely,

    Dr. Katharina Spies

    Dear Dr. Spies,

Should the KirGene also be tested in the case of a single donation if you have a child with your partner? Can you still be incompatible? Is there also a possible incompatibility between donor and recipient?

    Kind regards! Oskar

    Hello Oskar
Yes, of course. Most of our patients who undergo egg donation treatment do so together with their partner, and if indicated, we check the KIR - HLA-C.

On the one hand, we have the embryo, which is always the result of the combination of male and female gametes, and there may be an incompatibility between the donor and the partner. And then there is the pregnancy, for which the mother and the embryo must also be compatible. Whether it is an egg donation or a double donation does not change this process.

I hope this answers your question,
    Yours sincerely
    Dr. Spies

    Hello Dr. Spies,

I (47) am about to start EZS treatment with my husband (50).

I was tested for AA for killer cells. My husband was tested for HLA-C with the result C1,C2

The donor (26 years old) was also tested for HLA-C and also received a result of C1, C2.

Would the donor now be compatible with me, the recipient?

The doctor originally said that an optimal donor for me should have C1,C1 or C2,C2.

What is your opinion on this?

I would be very grateful for your feedback on this.

    Thank you very much

    Sunny

    Dear Sunny12,

In your case, you need a C1-C1 donor. Since your husband is C1-C2—which means that even with a C1-C1 donor, you have the possibility of receiving a C1-C2 embryo— you will also need an appointment with an immunologist, preferably one who specializes in fertility, so that they can prescribe the right treatment to prepare your uterine lining.

    I hope this helps,

    Best regards,

    Dr. Spies.
