    Hello,
We’ve been trying unsuccessfully to conceive a second child for the past 1 1/2 years. My son was born via C-section. During a laparoscopy 6 weeks ago, a niche was discovered in the C-section scar. Two very small endometriosis lesions on the ovaries were also removed. Should we have the niche corrected first, or should we try artificial insemination first? My husband’s first semen analysis showed limitations in all parameters at the first level, while the second one was normal.

    Dear EMJ,
Thank you very much for sharing your story with us.
Without a complete report on your laparoscopy, it’s difficult to give you a definitive answer, but there is indeed a risk that the embryo could implant in that cavity.
However, given how long you’ve been trying to get pregnant, your mild endometriosis, and the reduced sperm quality, my advice would be to skip intrauterine insemination (IUI) and start an IVF cycle right away. The success rates are significantly higher with IVF.
    I hope this helps,
    Best regards,
    Dr. Miranda Sanz

Thank you very much for the quick response. I understand that the risk of the embryo implanting in the niche is rather low. However, I’m wondering if the niche fundamentally prevents implantation. That would mean that artificial insemination wouldn’t make sense until the niche has been corrected. Or does it still make sense to try artificial insemination first? Are there any recent experiences with this issue?

    Hi EMJ,

I understand your concerns, but to assess the risk of impaired embryo implantation, the doctor who is familiar with your case and knows the size and location of your niche is best suited to answer this question.

IVF is always beneficial for embryo development (which is completely uncontrolled with IUI) and for preserving your fertility, so you won’t have to wait any longer as your egg reserve declines (due to endometriosis and age). Once you have good-quality embryos, you can have your endometrium prepared through a procedure if necessary. You do not have to have the embryos transferred immediately.

    Best regards,

    Dr. Miranda Sanz
