Hi, I’ve been going through hell for the past 4 days. On Tuesday evening (5+5), I had dark red bleeding and cramps. Whole blood clots came out. After a sleepless night, I went to the fertility clinic early Wednesday morning (5+6) (I’d had IVF). My hCG level was 8,400, and we could see a gestational sac. But a single test result doesn’t mean anything, and I had to wait until today. The bleeding stopped on Wednesday, and so did the cramps. Today during the ultrasound, they could also see an embryonic structure (3.5 mm). Unfortunately, a hematoma was also visible. So I was briefly reassured. But then the hCG level came back at 8,400. Unfortunately, it didn’t go any higher. Now, sadly, it’s back to waiting. But the doctor said the chances of a viable pregnancy are unfortunately very low. Now I’m having cramps again, but there’s hardly any bleeding. Is this going to be a miscarriage? Or could it just be the hematoma resolving? Is it even possible that the pregnancy is still viable if the hCG hasn’t risen?
    Thanks for your help.

    Dear Luna2107

Please excuse my late reply. Due to the summer break, I unfortunately didn’t receive your message until now.

The fact that the β-hCG level hasn’t risen within two days isn’t, of course, a particularly good sign at first. However, with an hCG level already this high, the rate of hCG increase is no longer the most important factor. What matters most now is the ultrasound findings—specifically, how the pregnancy is progressing and whether a fetal heartbeat is visible.

Since my reply is unfortunately reaching you with some delay, this should now be assessable via ultrasound. I would therefore recommend that you closely monitor the pregnancy’s progress with your fertility clinic.

If you experience heavy bleeding or pain again, please do not hesitate to contact your fertility clinic directly or, if necessary, go to the emergency room.

I sincerely wish you all the best and hope that the pregnancy continues to progress well.

    Warm regards
    Dr. Miranda Sanz
