Hormones do not work as expected

    Hello dear team of experts,

    About our conditions:

    We (W36 and M37) have been trying for over a year at the KiWu clinic (never pregnant before).

    My values: AMH value is 3.x, all other hormones are supposedly normal. Only diagnosis so far: follicle maturation disorder. A mild endometritis was successfully treated with antibiotics between the 2nd and 3rd IUI. The check-up was unremarkable. A hysteroscopy was also unremarkable. I still don't know why I often have intermenstrual bleeding after exercise.

    My husband: The spermiogram was always excellent.

    Previous path:

    We had 4 cycles of GV on schedule and 3 IUIs (each stimulated with Ovaleap, triggered with Ovitrelle). It was noticeable that my ovulation did not take place as calculated by the KiWu clinic, but was extremely delayed. Despite the fact that more than 36 hours had passed since the injection, there was still no ovulation. I had to inject a second ovitrelle for each IUI, and the egg did not hatch until a day later. So in total more than 50 hours after the first ovitrelle and one day later than the KiWu clinic calculated.

    Our 1st IVF (most recent attempt):

    Stimulation: again with Ovaleap and triggering with Ovitrelle, also Ganirelix.

    Ultrasound: 7 follicles and a cyst were seen on ultrasound.

    Puncture: The collection took place approx. 36.5 hours after the trigger injection.

    Result: Only one egg could be retrieved. Unfortunately, this could not be fertilized.

    My questions:

    What could be the reason that only one egg is found in the end with 7 follicles in the ultrasound?

    Were the other follicles really "empty" or could it be that the eggs had simply not yet detached from the follicle wall?

    Is the hcG working properly for me?

    Should I change the medication/protocol?

    If so, what should I do differently?

    I will do my best to answer your question here on the forum, but please keep in mind that I do not have your entire medical record, which may contain other details that you do not currently associate with your results, but could be related.

    What could be the reason that with 7 follicles on ultrasound only one egg is found in the end?

    This could be due to a number of factors. For example, the follicles may have been empty, the eggs may not have matured properly, the hCG test may not have worked correctly or there may have been premature ovulation.

    Were the other follicles really "empty" or could it be that the eggs had simply not yet detached from the follicle wall?

    It is not possible to say with 100% certainty. In my case, when I do an egg retrieval, I always check with the lab to see if we have an egg before I leave the follicle, and if not, I check again and "flush" it before I withdraw the needle, just to be sure.

    Is the hcG working properly for me?

    That is one of the possibilities, yes. That the usual dose is not triggering ovulation in your case, or that it is triggering it later than usual.

    Should I change the medication/protocol? If so, what should you do differently?

    Whenever we have a bad result in a cycle, we always consider changing the protocol before starting a new cycle. I'm not saying we will change it 100%, but in some cases like yours this should be considered, whether it's by adjusting the stimulation medication by allowing the follicles to grow larger than in previous stimulation cycles, double triggering with Decapeptyl and Ovitrelle, increasing the Ovitrelle dose or checking to see if ovulation occurs after the trigger.
