    Good morning, My hope....

about me: after the 3rd donation of a blastocyst (PGTa) 5BA I had a child in 2024. At that time TH1 balance was shifted and through Prograf 3mg, Granocyte, Intralipid I got pregnant. Ermap was 5.5 days. Previously in 2021 1 miscarriage in the 9th week EZS untested 5AA Blasto and otherwise 1 x no implantation. C-section with 1st child without complications, everything went well.

    Current situation:

Ermap 6.5 days (test repeated 3 times)

    Imap unremarkable

    Coagulation test unremarkable from 2023

Blood values OK so far, except for

    ......Ferritin 20 mg and TPO-AK 9.8 (TSH 1.8, Ft3 2.5ngl, anti TG 0.99, ft 4 0.76)

Progesterone at 25 ngl on transfer day, after that the progesterone drops very quickly every 2 days despite highest doses, blood sample is always taken 2h after Prolutex administration

    Medication:

3mg Prograf (from 10 days before transfer) prophylactically

20 mg prednisolone from transfer (10 mg before transfer) (previous transfer 10 to 15 mg)

Granocyte 34. Mio from transfer 0.3ml protocol

Intralipid on transfer day and after positive test

Ovitrelle this time on TR , TR+2, TR +4

    Ass 100 mg

    Folic acid 5mg

    Clexane 40mg

    Estradiol 10mg

    Prolutex 2 x 25 mg syringe,

progesterone 3x 300 mg vaginally from transfer

    B12, Lythroxin 88, iron 100mg, b6, k2, vitd3, etc....

Mucosa between 8.7 - 11mm during transfer. 3 layers. No polyps, fibroids....

I am already taking everything to cover everything and still no implantation. Not even biochemically. I am desperate.

I could only read from the forum that CD138 plasma cells/screening would still be an option. I would like to try to avoid surgical interventions as I am afraid that this could lead to adhesions. Is IVIG therapy perhaps another option? Pder Decapeptyl in the trial cycle?

I don't think it's due to the embryo, as a PGTa test has taken place. I have also already had an EMS and PGTa test, but there was no implantation.

This raises the question of what the next steps should be to avoid losing valuable embryos. Especially as the financial burden also has a limit...

    I'm hoping for some tips/help..... I'm desperate.

    Thank you!!!!

    Dear lauraba,
Thank you for this detailed information about your situation, and I am very sorry that you have had to endure such a difficult journey.
In your case, well... I should probably read your entire medical history carefully to make an informed judgment. You mention Ermap and Imap (from the iGLS lab): Here at Vida, we prefer the EndomeTrio (Igenomix), which can also test for endometrial infection and microbiota. CD138 and a hysteroscopy (done correctly by a professional to avoid adhesions) might also be good options, as well as a thrombophilia scan.
I wish I could help you more, but just via the forum is quite complicated.
    Kind regards,
    Dr. Spies
