    Hello,

after my last visit to the gynecologist felt a little unhelpful, I would appreciate a second opinion here.

First of all, I am 34 years old (but will soon be 35) and am still at the beginning of my fertility journey. I've actually always had a very regular cycle in my life and even without paying particular attention to it, I've always been able to observe the changes in my discharge quite well. However, I also usually had spotting towards the middle of my cycle, which I assume was related to ovulation.

A few months ago, my husband and I decided that we wanted to try to have a child, but I wanted to prepare myself for it first and it didn't quite fit in with my schedule to actively try to get pregnant right away. So for the time being I only took folic acid and job supplements and monitored my cycle a little more closely.

In August last year, I had a fungal infection (the first in my life) and treated it with a remedy from the pharmacy because I was on vacation at the time. After the infection and after taking the medication, my period was about 10 days late, but I had read that this can happen, but since then I have had the feeling that my cycle in general is no longer running "smoothly".

After the one delay, my period came on time again, but I couldn't see any real change in my cervical mucus and other symptoms were sometimes quite inconsistent or different to previous cycles, such as the bleeding between periods or PMS symptoms which varied from month to month.

I went to see my gynecologist last month about my desire to have children and actually wanted her opinion on these cycle changes, but I think she was having a bad day. She was very short tempered and didn't respond at all to what I had said about the cycle and sent me away with a recommendation for folic acid and iodine (which I already take anyway) and monk's pepper.

What is particularly unsettling for me now, however, is that I am actually in the middle of my cycle, but have had quite heavy bleeding for 2 days. At first I thought it was normal spotting, but it's too heavy and long-lasting for that. Could this be a very premature period? It doesn't actually feel heavy enough for that. The blood is also very dark and clumpy.

Could this still be related to the fungal infection or the medication, because I had the feeling that it all started with that, or was the infection perhaps just a symptom of another change that took place at the time?

Should I go back to the FA and insist more vehemently on answers and possibly tests or should I wait and see if it goes away on its own?

Thank you in advance for your answer.

    Dear PaperWing

Thank you for sharing your story with us.

First of all, I am very sorry that your gynecologist made you feel this way. I will never understand how a doctor can make a patient feel that way.

You are right to look for answers because 35 is a delicate age when it comes to fertility and even if there is no urgency, you should go ahead without hesitation and trust your own feelings about your cycle.

Another important point is that even if a woman is trying to conceive now, this does not mean that it will work immediately. It usually takes several months, even if there are no fertility problems.

If I were you, I would go back to my gynecologist (or find another one) and ask for a vaginal culture (+ a pap smear) to see if the infection has cleared up. See her at the beginning of your cycle while you are on your period so you can also ask for a fertility workup with antral follicle count and check your fertility hormones.

As for your mid-cycle bleeding, ovulation doesn't usually cause bleeding and I would also check by ultrasound that there is no fibroid or other uterine condition that could explain this.

These would be good initial tests. To be even more sure, I would then have your husband's sperm quality checked. If you have not conceived after 6 months, please see a fertility specialist.

    I hope this helps you.

    Yours sincerely,

    Dr. K. Spies.
