Laproscopy

    Dear team of doctors,

    I have an appointment for an operation next week to check whether I have a hydrosalpinx or not. The ultrasound was not able to clarify whether it is actually a hydrosalpinx or perhaps "just" a cyst.

    I now have to decide how the surgeon should proceed, as he can't wake me up and ask me in between...

    I don't want to give up my fallopian tubes, emotionally this is very unpleasant for me, because then a miracle can no longer happen (even if this is extremely unlikely).

    I would therefore like to tell the surgeon: "only if inflamed out" but does that make sense? What do I do if only one is infected?

    Background: I am planning an egg donation. I have never had chlamydia and no endometriosis.

    Dear Oskar,

    From what you tell me, it is a "diagnostic" laparoscopy to determine whether it is hydrosalpinx or not, but also to determine the degree of infection etc. In principle, it should not be a "surgical" laparoscopy at the same time, and you should be able to discuss the results with your doctor after waking up before making a decision.

    For example, if you want to do diagnosis and treatment in a single procedure for economic or time reasons, I would advise you to weigh up ALL the possible options with your doctor beforehand: depending on the degree of infection, whether one or two fallopian tubes, depending on your chances of becoming a mother with your own eggs, depending on the cause of the infection (it's not because you've never had lumps before or that you've never been diagnosed with endometriosis, which you don't have now), etc.

    From what you tell me, you are in the process of egg donation, but I don't know too many parameters of your case to give you more specific advice.

    A natural pregnancy is extremely unlikely for me, but women with an unfulfilled desire to have children will probably never be able to give up the last spark of hope... So the fallopian tubes are actually "superfluous" and could also be radically removed, I just can't really cope with it emotionally.

    Diagnosis and treatment are to be carried out in a LKS, I don't know whether it would be possible to get two appointments for diagnosis and then treatment. As I will be off work for 8 days afterwards (I was told), the LKS is probably not a "walk in the park", is it?

    The surgeon therefore wants "instructions" from me in advance as to what he should do in which case.

    I think that anything that is diseased should be removed (regardless of the desire to have children), but how can he see this during an LSK? You only look at the fallopian tubes with a laproscope from the "outside", right?

    P.s: My gynecologist has ruled out inflammation with a smear and blood test, she suspects endometriosis that has occurred "with age"... Is that possible? Perhaps due to the extreme hormone treatments plus bleeding after my last three egg donation transfers?
