Poor embryo development - egg donation?

    We have had everything checked (hormones, KIR, coagulation, immunology, DNA fragmentation, GMSH biopsy, GM endoscopy, laparoscopy....) but although we always have lots of eggs that fertilize well and I am young (32), hardly any blastocysts develop from our embryos. Many remain at the morula stage. The blastocysts that do develop are always far too slow and only become blastocysts on day 6, but have not even expanded by then. On day 6 mostly 2AA, 2AB, 2BB etc. I had 6 transfers but never got pregnant. However, I did get pregnant naturally very quickly once (unfortunately ELSS with EL removal).
    We always stimulated with the short protocol, 5 stimulations in total.
    What else can be done?
    Would a change of protocol (long protocol) help?
    Or a low stimulation/IVF in the natural cycle?
    What do you think of PRP for quality?
    Possibly a TF on day 3, where all embryos are still good?
    What are the chances with a 2AA embryo on day 6?
    Should we already think about EZS?

    That's also okay, of course. Not the best, but certainly okay. Fragmentation (in the extended spermiogram) under 10%.

    My very personal advice - and I wish we had done it this way ...

    but you're always smarter afterwards ☺️!!!

    If I could go back in time, we would have exhausted the health insurance options here in Germany.

    As soon as we would have been "self-payers", we would have moved abroad !!!!!

    Only there can the embryos be genetically tested to see what's really going on with the quality.

    In my case, it would probably have turned out that my eggs had been very bad for many years (and were similar in age to yours).

    We have spent a lot of money here in Germany - I don't want to know how much - and ended up with no results!!!!!

    I wouldn't think about egg donation at your age yet - but I would want clarity!!!

    Dear patient,

    Such a development (good development until day 3 then no more) could indicate sperm quality problems. If an egg quality problem is indeed suspected, luteal phase stimulation might be an option. Regarding the AA embryo, it should have a good chance.

    Dear Dr. Izquierdo!

    Thank you very much for your answer. I thought luteal phase stimulation was more for poor responders or cancer patients. Can it also improve quality (not just quantity)?

    Do you have any other ideas?
