Low HCG value in the blood test

    Dear team of experts,

    Yesterday I had a transfer (Cryo-Blasto 5AA) plus 10 and today I received the result of a very low HCG of 16.2. I also had the progesterone value determined on my own initiative, which is 1727.6.

    The clinic has me take 25mg Prolutex subcutaneously in addition to 800mg Cyglogest daily and 8mg Estrifam, 100mg ASS and 4000 Clexane, as well as 5mg Folsan (MThfr).

    How is the value to be seen?

    Does the HCG indicate a biochemical SSW and can the SSW be optimized by additionally administering Prolutex? thank you very much in advance for your assessment.

    I can't really say anything about your values, but I hope very much for you.

    You probably won't get an answer here before Monday, what does your clinic say about the value? If they had no hope, they wouldn't give you a new medication plan, would they?

    Dear Oskar,

    Thank you for your reaction. I'm still freaking out today. My questions and concerns about biochemical SS have not been answered by my clinic, although I have asked specific questions. Maybe they can't do that either! I can pick up Prolutex straight away. It's a strange feeling - the result is pregnant, but it's questionable whether it will stay that way.

    I won't have any real certainty until Tuesday. The only positive thing is that the urine test (digital 1-2) showed yesterday. That reassures me a little - but unfortunately the previous experience has an emotional impact.

    Hello Tascha0205 I had an HCG of 17 in 2020 which ended in a biochemical SS. In my German fertility clinic you are considered pregnant from HCG 20. I'm keeping my fingers crossed for a miracle. If it makes you feel any better, I got really pregnant in the cycle after the 2020 biochemical pregnancy and have the most amazing son you could ever wish for! Unfortunately, I've already been through a lot when trying for a sibling. But I wanted to motivate you to keep going, to stay positive no matter how it turns out now. I don't know what the unit of measurement for progesterone is because I had much lower values - it has to be at least 10. But then it's a different unit of measurement

    ERMap auffällig-rezeptiv P +6,5, ImMap unauffällig, Mirobiologie und Pathologie unauffällig

    HSK im September: alles in Ordnung, unauffällig

    TF: 17.10.2020 schlüpfende 5BB Blasto PGS geprüft

    HCG 27.10: 18 schon wieder vorbei

    TF: 04.12.2020 schlüpfende 5BB Blasto PGS geprüft

    I suspect this is what it will come down to..... at the moment I'm more in a state of shock, fear, anger and sadness. One day I tell myself I'm fighting, another day I give up and think I'm living in an unattainable illusion.

    The HCG value is not a meaningful value for an ectopic pregnancy!
    You really only need to check it now.

    Thank you for your reply. Did I understand correctly that the value could be an indication of an ectopic pregnancy? So you are also assuming that the pregnancy is not intact?

    No, the other way around, the HCG value is NOT indicative of an ectopic pregnancy.

    Thank you very much for your renewed feedback. Unfortunately, I have known since Monday evening that the value has halved to 7.26 in the second blood test and that there was therefore another biochemical SS.

    I don't want to give up and we still have implantable embryos cryopreserved.

    However, I am very afraid of a repeat biochemical SS.

    From an expert's point of view, what would you advise as to which tests would be useful in order to achieve proper implantation that is clinically detectable?

    Examination for killer cells/plasma cells, hysteroscopy of the uterus? Or perhaps a change of donor? I wonder why I reject the embryos early. Do biochemical SS occur more frequently in succession?

    I don't think it's due to the implantation window.

    The GMSH was three-layered and optimally structured for both transfers.

    I would recommend an examination for killer cells/plasma cells and a hysteroscopy of the uterus.

    I would not advise a change of donor. In this case, I would test for KIR genes to see if a drug therapy would be advisable.
